Health at Home provides comprehensive home health care services to support patients recovering from illnesses or surgery or managing a disease. We do this in partnership with families, hospitals, and physicians with the common goal to provide care, comfort, and support to the patient.

Our Services:

  1. Medical Care

    • Consultation with a doctor at the convenience of your home

    • The GP doctor will visit the patient at home, perform a medical assessment and consultation, recommend tests & treatment plan, or alternatively, refer the patient to the relevant specialist.

  2. Nursing Care

    • Nursing care services are provided at your home through our team of trained & licensed nursing staff. Our services include:

      • Observation and Assessment

      • Wound Care Management

      • Medical Management (IV and IM Meds)

      • Pain Management

      • Ostomy Care and Instruction

      • Care of Central Venous Catheters

  3. Blood Sample Collection

    • Our nurses/lab technicians will visit your home to collect lab samples anytime between 6 am to 8 pm.

  4. Physiotherapy & Rehabilitation

    • Physiotherapists visit your home and guide you therapeutic exercises recommended by the doctor

    • Ideal for patients recovering from surgery or those who cannot visit the hospital

  5. Chronic Disease Monitoring

    • Specially designed program to help patients diagnosed with chronic diseases, effectively manage their health

    • Nurses visit your home on a schedule to check vitals, makeassessments, and ensure compliance with medication, diet, and exercise.

  6. Medication Education and Management

    • Clinical Pharmacists provide instructions and directions (when, why and how) on the usage of prescribed medication.

  7. Welcome Baby

    • Licensed Nurses to visit new mothers in the comfort of their homes to support and counsel on breastfeeding, stump care & circumcision care.

    • Assistance is also provided to understand and learn personal care for baby dressing and bathing.
